If you wish to use our church's facilities we require everyone to fill out a Use of Facilities Form. There are two seperate forms: one for church members and/or the Baptist Association, and the other for non-members.

PLEASE NOTE: No form is complete until all sections of the form are answered and no activities are final until they are approved by the staff on Monday mornings. This process may take up to 3 weeks so please be courteous and fill out the form in a timely manner. The contact person on the form will be notified for either status approval or if the specified conditions cannot be met.  Should details change concerning your event please fill out another Activity Form to alleviate any scheduling problems that could occur.
